Questionnaire

1. Must be a Badger, Lake Geneva, or Genoa City High School graduate.
2. High school athletic accomplishments will be considered including:
   1. Number of sports participated in.
   2. Number of letters earned.
   3. Number of times was All-Conference.
   4. All-District or All-State recognition.
   5. Was a state champion (individual or team).
3. Post-Secondary athletic accomplishments will be taken into consideration.
4. If an athlete is outstanding in a sport or sports not offered at the high school; it is entirely possible for him/her to be inducted into the AWOF.
5. Must have been a graduate for at least 10 years.
